   Feature 014 (#14282) - T043 REVERSED: Kendo UI vs Bootstrap 5 box model
   ===========================================================================
   kendo.common.min.css sets `box-sizing: content-box` on `.k-widget`,
   `.k-widget *` and `.k-animation-container *`. Kendo stays frozen at
   2018.1.117 / Kendo.Mvc 2019.2.619.545 - the first Bootstrap-5-aware Kendo
   theme is 2021 R1, a licensed breaking upgrade across 184 views and out of
   scope for #14282 - so that declaration is not going away.

   T043 originally forced border-box back onto application markup inside Kendo
   containers, reasoning that it should match identical markup elsewhere on the
   page. That optimised for internal consistency, and this migration's
   acceptance bar is fidelity to the pre-migration rendering instead. Under
   Bootstrap 2 the baseline had no universal box model, so Kendo's content-box
   governed unopposed and every input inside a Kendo Window was content-box.
   The override changed that, and the Account Manager's fields were reported as
   visibly too small. Measured, app markup injected into a live `.k-widget`:

       .input-small    baseline 104x30 content-box    ->  T043  90x20 border-box
       .input-medium   baseline 164x30 content-box    ->  T043 150x20 border-box

   So the rule is removed rather than rewritten: Kendo's own content-box
   declaration reproduces the baseline exactly, and nothing needs to assert it.

   Grid spans need no rule either, and never did. Bootstrap 2 declares
   `.row-fluid [class*="span"] { box-sizing: border-box }` at specificity 0-2-0,
   which already beats Kendo's `.k-widget *` at 0-1-0 - measured identical at
   292px border-box on both sides, with and without this block. T043 was wrong
   for inputs and redundant for the case it was written to protect.

   If a Kendo-hosted layout ever genuinely needs border-box, scope it to that
   view rather than reintroducing a blanket rule here.

/* ===========================================================================
   Feature 014 (#14282) - T082: Provider portal brand and sidebar, widened 10px
   ===========================================================================
   Font Awesome 6 ships `fa-hospital` as an alias of `fa-hospital-wide`. At the
   brand's 20.4px it renders 25.5px wide against Font Awesome 3's 16px, and the
   Provider brand's content box was exactly 150px:

       fa-hospital 25.5 + space + "Provider Portal" 109 + space + caret 13
           = ~155.5px  against 150px available

   So the caret wrapped to a second line and the navbar rendered 65px tall
   against the baseline's 45px, pushing the whole page down 20px.

   The decision (Eric Romero, 2026-09-13) was to keep the icon rather than
   substitute a narrower glyph - fa-square-h and fa-building both fit but change
   the pictogram - and to widen the brand to a 160px content box instead. The
   sidebar has to move with it: its right-hand border is what the navbar brand's
   right edge lines up against, and they were both 190px by construction
   (150 + 20 + 20 padding = 190; 189 + 1px border = 190).

   New alignment, both 200px:
       brand 160px content + 40px padding (set inline in _Layout.cshtml)
       sidebar  199px + 1px right border

   SCOPED to .provider-portal, a class this feature adds to the Provider
   layout's <body>. The hospital portal keeps 190px - its brand is
   `icon-desktop` + "ClearPath", which still fits - and the two portals do not
   need to agree with each other, only each with its own sidebar.

   These override the generated Content/cb-compat/_ace.css (.sidebar 189px at
   :1957, .sidebar:before 189px at :1966, .main-content margin-left 190px at
   :68, .breadcrumbs-fixed left 190px at :1766). That file is regenerated from
   the Ace sources and must not be hand-edited, which is why the overrides live
   here instead.
   ------------------------------------------------------------------------ */
.provider-portal .sidebar,
.provider-portal .sidebar:before {
  width: 199px;
}

/* #14792: the rule above ties Ace's `.menu-min.sidebar { width: 42px }` on
   specificity and loads later, so the collapsed sidebar kept 199px while
   `.menu-min.sidebar + .main-content` still dropped the margin to 43px and the
   content slid under the float. Restore Ace's collapsed rail width. */
